Completion of this form puts you in the draw to win a Trek Marlin 4 Mountain Bike!

The T&Cs are:

• The prize is 1x Trek Marlin 4 mountain bike valued at $799.99
• One prize available per Game On series Location
• Colour and size subject to availability
• Prize must be collected from a Trek store in Canberra (Phillip, Fyshwick or Majura Park)
• Prize is not transferrable and cannot be taken as cash
• Trek reserves the right to substitute with similar product 
• Eligible entrants are those who fill out the suburb-specific event feedback forms
• Entrants can leave feedback once per household, per event (max four times across a suburbs event series).
• Surveys filled for events not yet held will not be eligible to win.
• Winners from any previous events are not eligible to win at any further events in the same calendar year.
• The winner of each suburb-series prize will be randomly drawn two days after the last event in that suburb at 5pm.
• The winner will be notified by email that same day and they must respond to organise collection within 7 days of the draw otherwise an alternate winner will be drawn.
• The winner’s name (supplied in the survey form) will be announced on the Physical Activity Foundation’s FB page upon email response from the winner
