18/25 June & 2 July 2025 – Kambah Adventure Park Evaluation Form

Completion of this form puts you in the draw to win a Digital Gift Card for Ninja Parc, Fyshwick OR one of two Digital Family Passes for Flip Out, Majura Park available for fun family experiences – perfect for the school holidays!!

The T&Cs are:

• The prizes are digital gift cards/family passes for use at Bounce, Westfield Belconnen, Ninja Parc, Fyshwick or Flip Out, Majura Park valued at up to $100 each, subject to the T&Cs of each business e.g. some require mandatory grip socks (e.g. at families’ cost, waiver completion before participation).
• Three prizes available per Game On Series location/suburb; they are not transferrable and cannot be taken as cash.
• Eligible entrants are those who fill out the Game On location/suburb-specific event feedback forms – can leave feedback once per household, per event (max three times across a location/suburb’s Game On Series).
• Feedback forms completed for events not yet held will not be eligible to win.
• Winners from any previous events are not eligible to win at any further events in the same calendar year.
• The winners of each Game On Series location/suburb prizes will be randomly drawn two days after the last event in that location/suburb by 5pm.
• The winners will be notified by email that same day and must respond within 7 days of the draw otherwise alternate winners will be drawn.
• The winners’ names (supplied in the feedback forms) will be announced on the Physical Activity Foundation’s Facebook and Instagram pages upon email response from the winners. If possible, each winner will share an image of their children having fun using the digital gift card/family pass for the Physical Activity Foundation to share on its social media channels.
