Completion of this form puts you in the draw to win a Family Pass for Flip Out Majura OR a Physical Activity Foundation Move It Pak for the park, beach or travel – perfect for the school holidays!
The T&Cs are:
- Eligible entrants are those who fill out the Game On school/community location feedback form – you can leave feedback once per household, per session
- Feedback forms completed for events not yet held will not be eligible to win.
- Winners from any previous events are not eligible to win at any further events in the same calendar year.
- The winners of each Game On Series school/community location prizes will be randomly drawn two days after the last event in that school/community location by 5pm.
- The winners will be notified by email that same day and must respond within
7 days of the draw otherwise alternate winners will be drawn. - The winners’ names (supplied in the feedback forms) will be announced on the Physical Activity Foundation’s Facebook and Instagram pages upon email response from the winners. If possible, each winner will share an image of their children having fun using the Family Pass or Move It Pak for the Physical Activity Foundation to share on its social media channels.
